How to add Document & Form Templates?
Document Templates in Helix are used to create and manage important documents and forms that need to be stored in the EMR for record-keeping purposes.
Summary
Helix supports both types of templates:

Document Templates (Sign Forms)
Used to upload PDF documents that require signature capture, such as Terms & Conditions or registration documents (excluding consent forms). -
Form Templates (Fill Forms)
Used to create digital forms directly inside Helix, such as registration forms or questionnaires, to be filled by patients or medical staff.
Both template types are configured from the Control Panel and later accessed from the Patient File.
🖊️ Document Templates — Sign Forms (Signature Capture)
What is it?
Sign Forms are PDF-based templates where predefined placeholders are placed on a document and later signed electronically in the patient file.

Control Panel → Templates → Document Templates → Document Templates (tab)
➕ How to Create a Sign Form Template
Step 1: Start Template Creation

Click + Add (top-right)
Step 2: Enter Header Details

Label
Enter a name for the form (e.g., Surgery T&Cs) -
Branch Selection
Multi-select the branches where this form should be available
Step 3: Upload the PDF

Upload the PDF file for the form
-
If the PDF has multiple pages:

Use the green arrows (top-left) to navigate forward or backward between pages

Step 4: Insert Placeholders

Open the Placeholder dropdown

Select a data field (e.g., Patient Name, Date, National ID)

Click directly on the PDF where the placeholder should appear

Repeat for all required placeholders
Additional options
-
Undo (top-right) removes the last placed placeholder

To add a new placeholder not in the list:

Type the field name in the dropdown
-
Click Add Item
-
Place it on the PDF
-
Step 5: Save the Template
-
Click Submit
Availability
-
No logout is required
-
The template becomes immediately available under Forms → Sign in the Patient File

Choose the template from the dropdown

Sign the document

Click Submit
View Signed Documents

📝 Form Templates — Fill Forms (Built Inside Helix)
What is it?
Fill Forms are digitally created forms built directly within Helix.
They are used for structured data capture such as questionnaires or registration forms.

Control Panel → Templates → Document Templates → Form Templates (tab)
➕ How to Create a Fill Form Template
Step 1: Enter Header Fields

Name
Clear and descriptive form title -
Description (optional)
Internal note about the form’s purpose -
Department
Select the department that will use this form -
Branch Sharing

Multi-select specific branches, or
-
Enable All Branches to share across the organization
-
Step 2: Add Groups (Sections)

Click + Group
-
Enter a Group Name (e.g., Patient Details)
Groups are used to organize fields into logical sections on the final form.
Step 3: Add Fields to a Group

Inside the group, click + Field
-
Select a Field Type:
-
Text – free-text input
-
Number – numeric input
-
Boolean – checkbox (Yes / No)
- Date picker - for documenting dates

Single-Select List – one option from a dropdown
-
Multi-Select List – multiple options from a dropdown
-
Radio Buttons – single choice displayed as radio buttons
- Image - image uploads

- Toggle Requried button if its mandatory
Values Box
(Visible only for list or radio field types)

Enter options separated by commas
Example: Mild, Moderate, Severe
Click Save Field

Repeat until the group is complete
Step 4: Arrange & Repeat

Add more groups and fields as needed
-
Reorder fields or groups for clarity
Step 5: Save the Template

Click Submit
Availability

The Fill Form becomes available under Forms → Fill in the Patient File
View Filled Documents

Filled templates appear in the Filled Forms table
📂 Where Templates Are Used

Sign Forms → Patient File → Forms → Sign

Fill Forms → Patient File → Forms → Fill
All completed forms are stored in the patient’s record for reference.
